‘Connect with a Coach’/Alumni Directory Release Form

 

I give, with pleasure, Newfield Network the absolute right to use, exhibit, publish, and display my name, all information I have provided, and any updates I may provide in the future, as part of the ‘Connect with a Coach’ & Alumni directory on the Newfield Network website and also to update the Newfield Network CRM.

 

I am aware that I may opt out at any time by following the opt out procedure.

 

I agree to waive the right to inspect and approve the finished product before it’s publication. 

 

I hold harmless and release Newfield Network from all claims, demands, and causes of action which I or anyone else acting on my behalf may make for publishing my information in the alumni directory.

 

I agree that any disputes that may arise under this release shall be governed in the courts of Colorado applying Colorado law. 

 

I confirm that I am at least 18 years of age and am competent to contract in my own name. I have read this release before signing below and I fully understand the contents and meaning. 

 

I sign this release voluntarily and without any expectation of compensation from Newfield Network.
